Baby teeth act as natural placeholders for the adult teeth developing beneath them. When a baby tooth is lost too early because of decay or injury, nearby teeth can drift into the gap and block the adult tooth from coming in properly. A space maintainer is a small appliance that holds that space open until the permanent tooth is ready to erupt.
Windham Centre parents whose child has lost a baby tooth early can ask our Port Dover dentists whether a space maintainer is needed. The dentist will look at which tooth was lost, your child’s age and the position of the adult tooth before recommending whether an appliance or careful monitoring makes more sense.

When it is needed
Space maintainers are most often considered when a baby molar is lost well before the adult tooth is due. Back teeth are particularly likely to shift forward into the gap. The dentist will take X-rays to see how far the adult tooth has developed and how long it may take to come in, which helps them judge whether holding the space is worthwhile.
If the adult tooth is close to erupting, or the lost tooth is a front tooth where drifting is less likely, the dentist may suggest monitoring instead of an appliance. Each situation is different. The dentist will explain the reasoning behind their recommendation, what to watch for and when the space should be checked again at a future visit.
Types of maintainers
Fixed space maintainers are cemented to a neighbouring tooth and use a small wire loop to hold the gap open. Because they stay in place, they cannot be lost or forgotten, which makes them practical for younger children. They need careful cleaning around the band and wire, and children should avoid sticky foods that could loosen or bend the appliance.
Removable space maintainers look similar to a retainer and can sometimes include an artificial tooth for appearance. They may suit older children who can reliably wear and care for them. Because they can be taken out, they must be worn as directed to be effective. The dentist will recommend the type that suits your child’s age, the gap and their habits.
Getting fitted
The dentist takes an impression or digital scan of the area so the space maintainer can be made to fit your child’s teeth. In some cases, a band is fitted around a neighbouring tooth during the first visit. The process is generally comfortable and does not usually require anaesthetic, although the team will explain each step in child-friendly terms as they go.
At the fitting appointment, the appliance is placed and checked for comfort and fit. Fixed maintainers are cemented in place, while removable ones are adjusted and your child is shown how to put them in and take them out. It may feel strange at first, but most children adapt within a few days and forget it is there.
Care and follow-up
Help your child brush carefully around the appliance and the tooth it attaches to, since plaque can collect around bands and wires. Avoid sticky candies, gum and hard foods that might loosen or bend it. Discourage pushing on the wire with the tongue or fingers. If you notice the appliance is loose, broken or causing soreness, call the office promptly.
The dentist will check the space maintainer at regular visits and take X-rays to monitor the adult tooth’s progress. Once the permanent tooth begins to erupt, the appliance is removed. Keeping up with checkups ensures the maintainer is taken out at the right time and that your child’s developing bite continues to be monitored carefully.
Planning your appointment
Call our Port Dover office if your child has lost a baby tooth early because of decay or injury. Windham Centre parents can describe which tooth was lost and when, so the dentist can assess whether a space maintainer is needed.
Bring any recent X-rays or records from other dental visits, and mention any habits such as thumb sucking that might affect the space.
Before you book
Have a question about your situation? Our team can help you plan a visit.
If the adult tooth is still a long way from coming in, nearby teeth may shift into the gap and block it. A space maintainer holds the room open. The dentist will check the X-rays to decide whether waiting is reasonable or risky.
Fitting is usually comfortable and does not normally require anaesthetic. Your child may feel some pressure or notice the appliance with their tongue at first, but most children adjust within a few days and forget about it.
The appliance stays in until the adult tooth begins to erupt, which can range from several months to a few years. The dentist monitors progress with X-rays at regular visits and removes the maintainer at the appropriate time.
Contact the office promptly. A loose or broken appliance may no longer hold the space properly and could irritate the gums. Keep any pieces and avoid letting your child play with it until the dentist can check it.
Most foods are fine, but sticky candies, gum and very hard foods should be avoided, as they can loosen or bend the appliance. Cutting harder foods into small pieces helps. The team will give you a list of foods to watch out for.
Crowding, crossbites or habits can affect how a child’s jaw grows. Windham Centre parents can learn what an early orthodontic assessment covers in Port Dover.
