Baby teeth hold space in the jaw for the adult teeth that will replace them. If a baby tooth is lost early because of decay or injury, neighbouring teeth can drift into the gap. This can block the adult tooth from coming in properly, sometimes leading to crowding. A space maintainer is a small appliance that keeps that space open.
Walsh families can have their child assessed for a space maintainer at our Port Dover office. The dentist will check which tooth was lost, when the adult tooth is expected and whether a maintainer is needed, then explain the options and how care works at home.

Is one needed
The dentist considers which tooth was lost, your child’s age, how far along the adult tooth underneath is and how the bite looks overall. Losing a baby molar early is one of the most common reasons for a maintainer, because back teeth tend to tip and drift forward into an empty space surprisingly quickly, sometimes within months.
If the adult tooth is already close to coming through, or if the lost tooth is a front baby tooth, a maintainer may not be needed at all. X-rays help show how developed the adult tooth is and how long it is likely to take. The dentist will explain the reasoning behind the recommendation, whichever way it goes, and answer your questions.
Types
Fixed maintainers are cemented to one or more teeth and include a thin wire loop or bar that holds the space open. Common types include the band-and-loop design for a single gap and the lingual arch, which runs behind the lower front teeth. Because they stay in place, children do not need to remember to wear them, which suits many families.
Removable maintainers look similar to retainers and may include artificial teeth to fill visible gaps. They can be taken out for cleaning, but they must be worn as directed to work, which depends on the child’s cooperation. The dentist will recommend the type that best suits your child’s age, mouth, habits and the particular tooth that was lost.
Fitting
The dentist may try small metal bands on the supporting teeth and take impressions or a scan. The maintainer is then made to fit precisely. Freezing is rarely needed for this step. At home, you might explain to your child that the dentist is making a special helper to save space for their big tooth until it is ready to come in.
At the next visit, the maintainer is cemented or fitted and checked for comfort and bite. Your child may notice it with their tongue for a few days and may feel slight pressure at first. Ask about scheduling these visits so they fit around school and the trip from Walsh, and so the gap between appointments is not too long.
Care
Brush carefully around the maintainer and the supporting teeth, since food can easily get trapped under the bands and wires. Children usually need an adult’s help to clean these areas well each night. Removable maintainers should be brushed daily and stored safely in their case whenever they are not being worn, so they are not lost or stepped on.
Avoid sticky candy, gum and very hard foods that can loosen or bend the appliance. Discourage pushing or pulling on it with fingers or the tongue. Call the office if the maintainer becomes loose, broken or uncomfortable. It is removed by the dentist once the adult tooth starts to come through, which is checked at regular visits.

If a baby tooth is lost early, neighbouring teeth can drift into the gap. A maintainer holds the space so the adult tooth can come in properly, reducing the risk of crowding or a blocked tooth that may need orthodontic help later.
Usually not. Fitting involves placing bands or taking impressions, and freezing is rarely needed. Some children notice mild pressure or feel the appliance with their tongue for a few days while they get used to it.
Until the adult tooth begins to come in, which can be months or years depending on age and the tooth. The dentist monitors progress with regular checkups and X-rays and removes the maintainer at the right time.
Call the office and keep the appliance if it comes out. A loose maintainer may need to be recemented or replaced to keep doing its job, and to prevent irritation of the gums or cheeks while your child waits.
Most foods are fine. Avoid sticky candy, gum and very hard foods that could damage it. Cutting food into smaller pieces may help at first while your child gets used to the appliance and learns to chew around it.
