Caring for Townsend patients at our Port Dover office
Every dental service for TownsendHolding room for adult teeth
Baby teeth hold space for the adult teeth developing underneath. When a baby tooth is lost early because of decay, injury or extraction, neighbouring teeth can drift into the gap, leaving too little room for the adult tooth. A space maintainer is a small appliance that keeps the space open until the permanent tooth is ready to erupt.
If your child in Townsend has lost a baby tooth earlier than expected, our Port Dover office can assess whether a space maintainer is needed. The dentist will look at which tooth was lost, your child’s age and the position of the developing adult tooth before recommending an approach.

Is one needed?
The dentist considers which tooth was lost, how soon the adult tooth beneath it is expected to come through, how much space is currently available and how the neighbouring teeth are positioned. X-rays help show the developing adult tooth and how far it has formed. Early loss of a back baby molar is more likely to need a maintainer than the loss of a front tooth.
If the adult tooth is already close to breaking through, or if the spacing is unlikely to change much, the dentist may suggest simply watching the area at future checkups. Ask the dentist to explain why a maintainer is or is not being recommended, and which changes you should look out for at home in the meantime, such as teeth tipping into the gap.
Types of maintainers
Fixed space maintainers are cemented to one or more teeth and cannot be taken out by the child. A common design uses a metal band around a neighbouring tooth with a wire loop that reaches across the gap. Fixed maintainers are often used for back teeth and are a reliable choice for younger children, who might otherwise lose or forget a removable appliance.
Removable maintainers look similar to a retainer and can be taken out for cleaning. Some carry artificial teeth to fill a visible gap, which can help with appearance and confidence. Because they only work when worn, they depend on the child keeping them in as instructed and looking after them, so they usually suit older, more responsible children.
Fitting the appliance
The dentist takes impressions or a digital scan of your child’s teeth so the maintainer can be made to fit the space precisely. For a fixed design, a band may be tried on the supporting tooth to find the right size. The process is generally painless and does not require any numbing, which helps keep the visit low-key for children who feel nervous.
At the fitting appointment, a fixed maintainer is cemented into place, or a removable one is tried in and adjusted. The dentist checks the fit and bite and explains how to keep it clean. Your child may notice the appliance with their tongue for a few days, and may talk a little differently at first, before getting used to it completely.
Caring for it
Help your child brush carefully around the band and wire, since food and plaque collect easily there and can lead to decay on the supporting tooth. Avoid sticky candies, toffee, gum and very hard foods that can loosen or bend the appliance. Gently discourage your child from pushing on it with their tongue or tugging at it with their fingers.
The dentist will check the maintainer at each regular visit and follow the progress of the adult tooth on X-rays when needed. Once the permanent tooth starts to come through, the maintainer is usually removed. Contact the office if it becomes loose, breaks, digs into the gum or causes discomfort, rather than waiting for the next scheduled checkup.

If a baby tooth is lost early, nearby teeth can drift into the gap, leaving too little room for the adult tooth when it arrives. A maintainer holds the space open until the permanent tooth is ready, which can help avoid crowding later on.
No. Fitting a maintainer is usually painless and does not need numbing. Your child may feel a little pressure while a band is placed, and will probably notice the appliance with their tongue for a few days before getting used to it.
It stays in place until the adult tooth begins to come through, which can range from months to a few years depending on the tooth and your child’s development. The dentist will monitor progress and remove it at the right time.
Contact the office promptly. A loose or broken maintainer may stop holding the space and could irritate the gum or be swallowed. Keep any pieces that come out and bring them along to the appointment so the dentist can see what happened.
Most foods are fine, but sticky candies, gum and very hard foods should be avoided, since they can loosen or bend the appliance. Encourage careful brushing around it after meals to keep the supporting tooth healthy while the maintainer is in place.
An early look at a child’s bite can flag issues worth watching. Learn what an orthodontic assessment covers and when monitoring or referral makes sense.
