Baby teeth act as placeholders for the permanent teeth growing beneath them. If a baby molar is lost early because of decay or injury, neighbouring teeth can drift into the gap and leave less room for the adult tooth. A space maintainer is a small appliance that holds that space open until the permanent tooth is ready to arrive, which may be years away.
Teeterville parents whose child has lost a baby tooth early can have the space assessed at our Port Dover office. The dentist will consider which tooth was lost, your child’s age and X-rays of the developing teeth before recommending whether a maintainer is needed or the gap can simply be watched.

Is one needed
Losing a baby molar well before the adult tooth is due is the most common reason for a space maintainer, because back teeth tend to shift forward quickly. Front baby teeth lost early often do not need one, since the space is less likely to close. The dentist will look at X-rays to see how far along the adult tooth is and how much time remains.
If the permanent tooth is close to coming through, or there is already plenty of room, the dentist may suggest monitoring instead. Your child’s overall bite and any crowding also influence the decision. Ask the dentist to explain the reasoning, including what could happen if the space were left without a maintainer for the coming months or years.
Types
A common fixed design uses a band or crown on a neighbouring tooth with a wire loop that rests against the tooth on the other side of the gap. Other fixed designs connect teeth on both sides of the arch. Fixed maintainers stay in place all the time, which suits younger children who might lose or forget to wear a removable appliance.
Removable maintainers look somewhat like a retainer and may include a replacement tooth. They can be useful when appearance matters or when several teeth are missing. Children need to wear them consistently and keep them safe. The dentist will recommend a design based on your child’s age, the teeth involved and how reliably they can wear an appliance.
Getting fitted
At the first visit, the dentist fits a band or takes an impression or scan of your child’s teeth. This is usually quick and does not require freezing. Explain the steps to your child beforehand in simple terms, and bring a comfort item if it helps. Ask how long the lab will need so you can plan the return trip from Teeterville.
At the second visit, a fixed maintainer is cemented in place, or a removable one is fitted and adjusted. The dentist checks the fit and bite and explains how to care for the appliance. Your child may feel some pressure or notice the wire with their tongue for a few days, but most adapt quickly.
Living with it
Brush carefully around the bands and wire, since food can collect there. Avoid sticky candies, chewing gum and very hard foods that could bend or loosen the appliance. Discourage your child from pushing on the wire with their tongue or fingers. For removable maintainers, clean the appliance daily and store it in its case when it is not being worn.
Regular checkups allow the dentist to confirm the maintainer is secure and to track the adult tooth on X-rays. Call the office if the appliance comes loose, breaks or causes soreness. Once the permanent tooth begins to come through, the dentist will remove the maintainer, which is usually a simple and quick procedure that needs no freezing.
Planning your appointment

Not always. It depends on which tooth was lost, how soon the adult tooth is expected and your child’s bite. The dentist will examine the area and X-rays and explain whether monitoring is enough or whether a maintainer is recommended to protect the space.
Fitting is usually quick and does not require freezing. Your child may feel some pressure or tightness for a few days while getting used to it. Most children adapt quickly and soon forget the appliance is there during daily activities.
It stays in place until the adult tooth starts to come through, which may be months or several years depending on age. The dentist will monitor progress with checkups and X-rays and remove the maintainer at the right time.
Call the office and keep the appliance if it comes out. Avoid letting your child chew on it or push it with the tongue. The dentist can usually recement or repair it at a short visit so the space stays protected.
Most foods are fine, but sticky candies, gum and very hard foods should be avoided because they can loosen or bend the appliance. Cutting harder foods into smaller pieces and brushing carefully afterward helps keep the maintainer secure.
