Baby teeth hold space for the adult teeth developing underneath. If a baby tooth is lost too early because of decay or injury, neighbouring teeth can drift into the gap, leaving too little room for the permanent tooth. A space maintainer is a small appliance that keeps the space open until the adult tooth is ready to erupt.
Families from St. Williams whose child has lost a baby tooth early, or needs one removed, can talk with our Port Dover team about whether a space maintainer is advisable and which type would suit the location and your child’s age.

When needed
Space maintainers are often recommended when a baby molar is lost early and the adult tooth will not appear for some time. Back teeth are particularly likely to shift into gaps. The dentist will consider which tooth was lost, your child’s age and how the adult teeth are developing.
If the adult tooth is close to coming in, or if the lost tooth was a front tooth where shifting is less of a concern, a maintainer may not be necessary. X-rays help the dentist see the position and development of the permanent tooth to make the right recommendation.
Types
Fixed maintainers are cemented to teeth and cannot be removed by the child. A common design uses a band around a neighbouring tooth with a wire extending across the gap. Fixed maintainers are often used for back teeth and for younger children who might lose or forget a removable appliance.
Removable maintainers resemble retainers and can be taken out for cleaning. They may include artificial teeth to fill visible gaps. They require the child to wear them consistently and look after them. The dentist will recommend the type that best suits your child’s needs.
Getting one
The dentist takes impressions or a scan of your child’s teeth and may fit a band around a neighbouring tooth. The appliance is made in a laboratory. Your child may notice some pressure when bands are fitted, but freezing is usually not required for this step.
At the fitting visit, the maintainer is placed and checked for comfort and fit. Fixed maintainers are cemented in place. Your child may feel the appliance with their tongue for a few days, which is normal. The dentist will explain how to care for it at home.
Care
Brush carefully around the appliance and the teeth it attaches to. Avoid sticky candy, gum and hard foods that can loosen or bend it. Teach your child not to push on the wire with their tongue or fingers. For removable maintainers, clean them daily and store them safely when not worn.
The dentist will check the maintainer at regular visits and watch for the adult tooth coming in. Once the permanent tooth begins to erupt, the maintainer is removed. Call the office if the appliance becomes loose, broken or uncomfortable, or if your child loses a removable one.
Planning your appointment
Call our Port Dover office if your child has lost a baby tooth early or needs an extraction. The team will arrange an assessment to see whether a maintainer is needed.
Plan for at least two short visits, one for measurements and one for fitting, and ask about the timing between them.

Neighbouring teeth may drift into the space, blocking the adult tooth from coming in properly. This can lead to crowding and the need for orthodontic treatment later.
Placement is usually comfortable. Your child may feel some pressure or notice the appliance for a few days. Persistent discomfort should be reported to the office.
Until the adult tooth starts to come in, which may be months or years depending on your child’s age and development. The dentist will monitor progress at checkups.
Call the office. Keep the appliance if it comes out and bring it with you. Avoid letting your child play with a loose appliance.
Most foods are fine, but avoid sticky, chewy and very hard foods that could loosen or damage the appliance. Cut food into smaller pieces if needed.
