Baby teeth hold space for the adult teeth developing underneath. When a baby molar is lost early, through decay or injury, neighbouring teeth can drift into the gap. This may leave too little room for the adult tooth, leading to crowding or a blocked tooth. A space maintainer is a small appliance that holds the space open until the permanent tooth is ready.
Families in Pine Grove whose child has lost a baby tooth early can have the space assessed at our Port Dover office. The dentist will explain whether a maintainer is needed or whether monitoring is enough.

When needed
Early loss of a baby molar, particularly when the adult tooth is still far from coming in, is a common reason for a space maintainer. The dentist considers which tooth was lost, the child’s age, the position of the developing adult tooth and how much space is available to decide whether holding the gap is important.
If the adult tooth is close to erupting, or the lost tooth is a front baby tooth, a maintainer may not be necessary. X-rays help show how close the adult tooth is. The dentist will explain the reasoning and what to watch for if monitoring is chosen instead.
Types
Fixed maintainers are cemented to one or more teeth and usually consist of a metal band with a wire loop extending across the gap. They stay in place until removed by the dentist. Fixed designs suit younger children who might lose or forget a removable appliance.
Removable maintainers resemble a retainer and may include artificial teeth to fill a visible gap. They need to be worn consistently and cleaned daily. They may suit older children who can manage them responsibly. The dentist will recommend the type that fits your child’s age and needs.
Fitting
The dentist takes impressions or scans of your child’s teeth and may fit a band to a nearby tooth. The appliance is made in a laboratory to fit the space precisely. The process is generally comfortable, and the team will explain each step in simple terms to your child.
At the second visit, the maintainer is checked for fit and cemented in place, or for removable types, your child is shown how to put it in and take it out. The dentist will give instructions on eating, cleaning and what to do if the appliance feels loose or uncomfortable.
Care
Brush carefully around the bands and wires to prevent plaque buildup and decay. Avoid sticky foods like toffee and chewy candy, which can loosen fixed maintainers. Discourage your child from pushing on the wire with their tongue or fingers. Clean removable maintainers daily and store them in a case.
The dentist checks the maintainer at regular visits to make sure it is secure and still needed. When the adult tooth begins to emerge, the maintainer is removed. Call the office if it becomes loose, bent, broken or causes soreness, so it can be repaired or adjusted.

Fitting is usually comfortable. Your child may feel some pressure or notice the appliance with their tongue at first. Most children adjust within a few days. Report persistent soreness to the office.
It stays until the adult tooth starts to come in, which may take months or years depending on age. The dentist monitors progress at checkups and removes it when appropriate.
Mostly, yes. Avoid sticky, chewy and very hard foods that may loosen or damage a fixed maintainer. Cutting food into smaller pieces can help.
Call the office promptly. Keep the appliance if it comes out completely. The dentist can usually recement or repair it, preventing teeth from shifting into the space.
No. A maintainer holds space but does not move teeth. Braces or other orthodontic appliances are used to move teeth into new positions. Your child may or may not need orthodontics later.
An early bite check can flag crowding, crossbites or habits while a child grows. Pine Grove parents can learn what is assessed and when referral helps.
Regular exams track how a child’s teeth and jaws develop. Pine Grove families can see what a checkup covers and how to prepare for a visit in Port Dover.
