Baby teeth hold space for the adult teeth developing beneath them. If a baby tooth is lost early because of decay or injury, neighbouring teeth can drift into the gap. This may leave too little room for the adult tooth, leading to crowding or teeth erupting out of position. A space maintainer is a small appliance that keeps the space open.
Families in Nixon can have their child assessed at our Port Dover office if a baby tooth has been lost early or needs to be removed. The dentist will explain whether a space maintainer is recommended, which type suits your child and how it will be monitored as they grow.

When it is needed
A space maintainer may be recommended when a baby molar is lost well before the adult tooth is ready to come in. Back teeth are particularly important for holding space. The dentist considers your child’s age, which tooth was lost and how far the adult tooth has developed, often using X-rays to see what is happening beneath the gums.
If the adult tooth is close to erupting, or if the lost tooth is a front tooth, a space maintainer may not be needed. In some cases, monitoring is enough. The dentist will explain why a maintainer is or is not recommended and what might happen if the space closes, so you can make an informed decision.
Types of maintainers
Fixed space maintainers are cemented to teeth next to the gap and usually consist of a metal band with a wire that holds the space. They cannot be removed by the child, which helps ensure consistent use. Fixed designs are common for back teeth. The dentist will explain how the appliance looks and what to expect while wearing it.
Removable space maintainers look similar to a retainer and can be taken out for cleaning. They may include artificial teeth to fill the gap. Removable designs require the child to wear them as instructed, which can be challenging for some children. The dentist will help you decide which type fits your child’s needs and habits.
Getting fitted
The dentist or team takes impressions or scans of your child’s teeth. The space maintainer is then made to fit. For fixed designs, small bands may be fitted on the supporting teeth first. The process is explained to your child in simple terms, and breaks are offered if needed. No freezing is usually required for fitting.
At the fitting appointment, the appliance is checked for comfort and fit, then cemented in place for fixed designs or adjusted for removable ones. Your child may notice it feels strange at first. The dentist will explain how to clean around it and what foods to avoid. Most children adjust within a few days and soon forget it is there.
Caring for it
Help your child brush carefully around the maintainer, especially where bands meet the teeth. Avoid sticky candies, gum and very hard foods, which can loosen or bend the appliance. Discourage pushing on it with fingers or the tongue. If the maintainer feels loose, bent or broken, contact the office promptly so it can be checked.
The dentist will check the maintainer at regular visits and take X-rays to monitor the adult tooth. When the adult tooth begins to come in, the maintainer is removed. Keep follow-up appointments so the timing is right. Removing it too late or too early can affect how the adult tooth settles into place in the arch.
Planning your appointment

Let the team know how the tooth was lost and whether your child has any habits, like thumb sucking, that might affect the appliance.

If a baby tooth is lost early, nearby teeth may shift into the gap and leave too little room for the adult tooth. A space maintainer holds the space open until the adult tooth is ready to come in, which may help reduce crowding.
Fitting is usually comfortable and does not typically require freezing. Some children feel pressure or notice the appliance for a few days. If soreness persists or the appliance rubs, contact the office for an adjustment.
It depends on when the adult tooth is expected to come in. The dentist monitors development with X-rays and removes the maintainer when the adult tooth begins to erupt. This can range from months to a few years.
Contact the office promptly. Keep the appliance if it comes out completely. A loose or broken maintainer may no longer hold the space properly and should be checked and recemented or replaced before the space starts to close.
Most foods are fine, but avoid sticky candies, gum and very hard foods that can loosen or damage the appliance. Cutting food into smaller pieces may help. The dentist will give specific guidance for your child’s appliance.
An early look at a child’s bite can spot crowding or jaw concerns during growth. Nixon parents can learn about timing, what is checked and possible next steps.
A child’s dental exam checks more than cavities. Nixon parents can see what the dentist looks for, how to prepare young children and how often to return.
