Port Dover Family Dentistry
When a baby tooth is lost early, a space maintainer can hold room for the adult tooth. Langton parents can learn when one is advised and how to care for it.

Baby teeth act as natural placeholders for the adult teeth developing beneath them. When a baby tooth is lost too early, through decay, infection or injury, neighbouring teeth can drift into the empty space. This can leave too little room for the adult tooth, increasing the chance of crowding, impacted teeth or more complex orthodontic treatment later on.
Langton parents whose child has lost a baby tooth early can have the situation assessed at our Port Dover office. The dentist will consider which tooth was lost, your child’s age, X-rays of the developing adult tooth and the spacing in the mouth, then explain whether a space maintainer is recommended or whether monitoring is enough.

When needed
Space maintainers are most often recommended when a back baby tooth, particularly a molar, is lost well before the adult tooth is ready to come in. Back teeth play a big role in holding space, and gaps there tend to close more quickly. X-rays help the dentist see how far the adult tooth has developed and estimate when it might erupt.
If the adult tooth is close to coming in, or if a front baby tooth is lost, a space maintainer may not be necessary. The dentist may simply monitor the gap at regular checkups. They will explain their reasoning and what signs would change the recommendation, such as teeth beginning to shift into the space faster than expected.
Types
Fixed space maintainers are cemented in place, usually with a band around a neighbouring tooth and a wire that extends across the gap. Common designs include band-and-loop maintainers for single spaces and lingual arch maintainers for multiple missing teeth. Because they are fixed, children cannot lose them or forget to wear them, which suits busy households well.
Removable space maintainers resemble small retainers and can be taken out for cleaning. They may include artificial teeth to fill visible gaps. They rely on the child wearing them as instructed, which works well for some children but not others. The dentist will recommend the type best suited to your child’s mouth, age and habits.
Fitting
At the first appointment, the dentist may fit a metal band around a neighbouring tooth to check the size and then take an impression or scan of the area. These are sent to a laboratory where the space maintainer is made to fit precisely. Explain the steps to your child in simple terms beforehand to keep them relaxed.
At the second visit, the maintainer is checked for fit and cemented in place, or, for removable types, adjusted and demonstrated. Most children feel some pressure at first and may notice the appliance with their tongue. These sensations usually settle within a few days, and your child can return to normal activities and school right away.
Daily care
Help your child brush carefully around the bands and wires, where plaque can collect. Avoid sticky candies, gum and very hard foods, which can loosen or bend a fixed maintainer. Discourage pushing or pulling on the wire with fingers or tongue. For removable maintainers, clean them daily and store them in a case when not being worn.
Regular checkups allow the dentist to make sure the maintainer still fits and the adult tooth is developing as expected. Call the office if the maintainer becomes loose, bent or broken, or if your child has pain or swelling. Once the adult tooth begins to erupt, the dentist will remove the maintainer at the right time.
Planning your appointment
If your child has lost a baby tooth early, call our Port Dover office to arrange an assessment. The dentist will check the space and the developing adult tooth and explain whether a maintainer is needed.
If a fixed maintainer comes loose, keep it safe and call the office rather than trying to put it back yourself.
Before you book

Placing a space maintainer is usually not painful. Children may feel pressure or tightness at first and notice it with their tongue. These sensations typically settle within a few days as your child gets used to the appliance.
It stays in place until the adult tooth begins to come in, which may be months or a few years depending on your child’s development. The dentist monitors progress with checkups and X-rays and removes it at the right time.
Without a maintainer, nearby teeth may drift into the gap, leaving less room for the adult tooth. This can lead to crowding or impacted teeth and may increase the need for orthodontic treatment later. The dentist will explain the risks for your child.
Mostly, yes. Avoid sticky foods, chewing gum and very hard foods that could loosen or damage the appliance. Cutting food into smaller pieces may help. Encourage your child not to play with the wire using fingers or tongue.
Contact the office if the maintainer becomes loose, bent or broken. Keep any pieces and avoid trying to fix it yourself. The dentist can repair or replace it so it continues to hold the space effectively.
An early bite check can spot jaw and spacing issues while a child is still growing. Langton parents can learn what is looked for and when to ask about referral.
