Baby teeth act as natural placeholders for the adult teeth developing underneath. When a baby tooth is lost too early, through decay, injury or extraction, nearby teeth can drift into the gap. This may block the adult tooth from coming in properly. A space maintainer is a small appliance designed to keep that space open until the permanent tooth is ready.
If your child has lost a baby tooth early, Glen Meyer parents can ask the dentist at our Port Dover office whether a space maintainer is recommended. The decision depends on which tooth was lost, your child’s age and how soon the adult tooth is expected.

When one is needed
Back baby molars help hold space for premolars, so early loss of these teeth is more likely to lead to drifting. Front baby teeth may not need a space maintainer in some cases, depending on the child’s age and spacing. The dentist will examine the area, consider how your child’s bite is developing and explain the reasoning behind any recommendation.
X-rays help show where the adult tooth is in its development and how long it may take to come in. If the permanent tooth is close to erupting, a maintainer may not be necessary. If it is still a long way off, holding the space may help prevent crowding or the need for more complex orthodontic treatment later.
Types of maintainers
Fixed maintainers are attached to one or more teeth with bands or crowns and a wire that spans the gap. They stay in place until the dentist removes them. Because they cannot be taken out, they work without relying on a child remembering to wear them. They do need careful cleaning, as food can collect around the bands and wire.
Removable maintainers resemble small retainers and may include artificial teeth for appearance or chewing. They can be taken out for cleaning but must be worn as directed to work properly. They may suit older children who can handle them responsibly. The dentist will explain which type is most appropriate for your child’s situation and age.
Getting it fitted
The dentist takes impressions or scans of your child’s teeth to create a maintainer that fits accurately. For a fixed maintainer, bands may be tried on the anchor teeth beforehand. Let the team know if your child has a strong gag reflex or sensory sensitivities, so they can plan ways to make impression-taking as comfortable as possible.
At the fitting appointment, the maintainer is checked for fit and comfort, then cemented in place or shown to you and your child if it is removable. Your child may feel some pressure at first or notice the appliance with their tongue. Most children adjust within a few days and stop paying much attention to it.
Caring for it
Brush carefully around the bands and wire of a fixed maintainer, and help younger children reach difficult areas. Avoid sticky or very hard foods, such as caramel, gum and hard candy, which can loosen or bend the appliance. Discourage pushing or pulling on the wire with fingers or the tongue. Contact the office if it becomes loose or damaged.
The dentist will check the maintainer at regular visits to ensure it still fits and is doing its job. X-rays may be taken to monitor the adult tooth. Once the permanent tooth starts to come in, the dentist will remove the maintainer. Keep up with appointments, so it is removed at the right time and does not interfere with eruption.

Have a question about your situation? Our team can help you plan a visit.
It depends on which tooth was lost, your child’s age and how soon the adult tooth is expected. The dentist will examine the area and X-rays to decide whether a maintainer is likely to prevent problems with spacing.
Most children feel only mild pressure or awareness of the appliance at first. Discomfort usually fades within a few days. Contact the office if your child has ongoing pain, sore gums or the appliance feels loose.
It usually stays in place until the adult tooth begins to come in. This could be months or longer, depending on development. The dentist will monitor progress at checkups and remove the maintainer at the appropriate time.
Call the office promptly. A loose or broken maintainer may not hold space properly and could irritate the gums. Keep the appliance if it comes out completely and bring it to the appointment for assessment.
Most foods are fine, but sticky or hard foods can damage or loosen the appliance. Encourage your child to cut food into smaller pieces and chew carefully. Brushing around the maintainer after meals helps prevent decay.
Crowding, crossbites or habits can affect how a child’s bite develops. Glen Meyer parents can learn what an early assessment checks and when referral may help.
