Baby teeth act as natural place-holders for the adult teeth developing underneath. When a baby tooth is lost early, through decay, injury or extraction, neighbouring teeth can drift into the gap. This may leave too little room for the adult tooth, leading to crowding or teeth coming in at an angle. A space maintainer is a small appliance that holds the gap open.
If your child has lost a baby tooth early and you live in Boston, our Port Dover office can assess whether a space maintainer would help. The dentist will look at which tooth was lost, your child’s age and how soon the adult tooth is expected to arrive.

When needed
The dentist considers which tooth was lost, how long until the adult tooth is expected and whether neighbouring teeth are already shifting. Back baby molars are more likely to need a maintainer than front teeth, since they hold space for larger teeth and affect the bite. X-rays help show the position and development of the adult tooth.
If the adult tooth is close to coming in, or the gap is unlikely to close, a maintainer may not be needed. The dentist may suggest monitoring instead. Ask about the reasoning, and what signs would change the recommendation over time. Regular checkups and occasional x-rays help confirm the adult tooth is on its way and that the space is holding.
Types
Fixed space maintainers are cemented to a neighbouring tooth and use a small wire loop to hold the gap. They stay in place until the adult tooth comes in and do not rely on the child to remember to wear them. Several designs exist depending on which tooth was lost and whether one or both sides are affected.
Removable maintainers resemble a retainer and can sometimes include a replacement tooth for appearance. They require the child to wear them consistently and keep them clean. The dentist will explain which type is most suitable for your child’s age, the missing tooth and how reliably they can manage a removable appliance at school and home.
Fitting
The dentist takes impressions or a scan of the area. For fixed maintainers, a small band may be fitted around a neighbouring tooth to check size. The appliance is then made to fit. The process is usually painless, though some children find impression material unusual. A simple explanation beforehand helps them feel prepared and cooperate.
At the next visit, the maintainer is tried in and, if fixed, cemented in place. The dentist checks the fit and bite. Your child may notice it with their tongue at first, and mild soreness can occur. Most children adapt within a few days. Soft foods for the first evening can make the change easier.
Care and removal
Brush carefully around the maintainer, as food can collect around bands and wires. Avoid sticky candies, chewing gum and hard foods that might loosen or bend it. Discourage your child from pushing on it with their tongue or fingers. Contact the office if it becomes loose, bent or uncomfortable, rather than waiting for the next checkup.
The dentist will check the maintainer at regular visits and monitor the adult tooth’s progress. When the adult tooth begins to come in, the maintainer is removed. This is usually quick and simple. Keep up checkups so the timing can be judged correctly. Removing it too early or too late can both affect how the adult tooth comes in.
Planning your appointment
Call our Port Dover office if your child has lost a baby tooth early. Mention which tooth and how it was lost, so the dentist can assess whether a space maintainer is needed.
If a maintainer becomes loose or damaged, keep it if it comes out and contact the office for advice on next steps.
Before you book
Have a question about your situation? Our team can help you plan a visit.
When a baby tooth is lost early, nearby teeth can drift into the gap, reducing space for the adult tooth. A maintainer holds the space open until the adult tooth is ready to come in.
Fitting is usually painless. Some children feel mild pressure or soreness for a few days. Most adapt quickly. Contact the office if discomfort persists or the appliance seems to rub on the gum or cheek.
Until the adult tooth starts to come in, which may be months or a few years depending on age and the tooth involved. The dentist will monitor progress at regular visits and tell you when it can come out.
Avoid sticky candies, chewing gum and very hard foods that could loosen or bend the maintainer. Cut foods into smaller pieces if needed, and encourage careful chewing. Check the appliance now and then for bent wires.
Contact the office. If it comes out completely, keep it and bring it with you. A loose maintainer may need recementing or replacing to keep holding the space. Try to call soon, since teeth can begin drifting.
