Saving room for adult teeth

A small appliance can help keep a child’s smile on track.

Baby teeth hold space in the jaw for the adult teeth developing beneath them. When a baby tooth is lost too early because of decay, injury or extraction, nearby teeth can drift into the gap. This can leave too little room for the adult tooth, leading to crowding or eruption problems later. A space maintainer is a small appliance that helps hold the gap open.

Bloomsburg families whose child has lost a baby tooth early can have the situation assessed at our Port Dover office. The dentist will check the space, the developing teeth and the child’s bite, explain whether a space maintainer is recommended and describe how it is fitted and cared for.

When needed

Not every early tooth loss requires a space maintainer.

Early tooth loss

A space maintainer may be recommended when a baby molar is lost long before the adult tooth is ready to come in. The concern is that neighbouring teeth may shift and reduce the space needed. Front baby teeth lost early may not always need one, depending on the child’s age and how the teeth are spacing in that particular area.

Factors the dentist considers

The dentist considers which tooth was lost, how old your child is, how far the adult tooth has developed and how the bite looks. X-rays help show the position of the developing tooth. If the adult tooth is close to coming in, a maintainer may not be needed. The dentist will explain the reasoning behind any recommendation clearly.

Types

Different designs suit different situations.

Fixed space maintainers

Fixed space maintainers are cemented to one or more teeth and usually include a band and a wire loop that holds the gap open. They stay in place until the adult tooth begins to come in. Because they are not removable, children do not need to remember to wear them, though they do need careful cleaning every day.

Removable options

Removable space maintainers look similar to retainers and can be taken out for cleaning. They may be used in certain situations, such as when several teeth are missing. They require the child to wear them consistently. The dentist will explain which type is best for your child’s needs and how to care for it at home.

Fitting

Fitting is usually simple and comfortable.

  1. Taking measurements

The dentist takes impressions or a scan of your child’s teeth to create a custom appliance. For fixed maintainers, a band may be fitted around a supporting tooth. The process is usually quick and does not require freezing. The team explains each step in child-friendly language so your child knows what to expect at each stage of the visit.

  2. Placing the appliance

At the fitting appointment, the appliance is placed and checked for comfort and bite. Fixed maintainers are cemented in place. Your child may feel some pressure or notice the appliance with their tongue at first. This usually settles within a few days. The dentist will explain how to clean around the appliance and which foods to avoid.

Care and follow-up

Good care keeps the appliance working until it is no longer needed.

  • Daily cleaning

Help your child brush around the appliance carefully twice a day. Food can get trapped around bands and wires, so a soft brush and gentle flossing are important. Avoid sticky, hard or chewy foods that could loosen or damage the appliance. Encourage your child not to push on it with their tongue or fingers during the day.

  • Regular checks and removal

Regular checkups allow the dentist to check the fit, look for loosening and monitor the developing adult tooth. Once the adult tooth starts to come in, the appliance is removed. Contact the office if the maintainer becomes loose, breaks or causes discomfort, so it can be checked promptly and adjusted or replaced if that is needed.

Space maintainer questions

Have a question about your situation? Our team can help you plan a visit.

Does my child need a space maintainer if a baby tooth falls out early?

Not always. It depends on which tooth was lost, your child’s age and how close the adult tooth is to coming in. The dentist will assess these factors and explain whether a maintainer is recommended for your child.

Is getting a space maintainer painful?

Fitting is usually quick and does not require freezing. Your child may feel pressure or notice the appliance at first, but this typically settles within a few days. The team explains each step to help your child feel comfortable.

How long will my child wear a space maintainer?

It usually stays in place until the adult tooth begins to come in, which may take months or longer. The dentist will monitor the developing tooth at regular checkups and remove the maintainer when it is no longer needed.

Can my child eat normally with a space maintainer?

Most foods are fine, but sticky, hard or chewy foods should be avoided, since they can loosen or damage the appliance. The dentist will give you a list of foods to avoid and tips for keeping the area clean.

What if the space maintainer comes loose?

Contact the office as soon as possible. Keep the appliance if it comes out completely. The dentist will check whether it can be re-cemented or needs to be replaced, so the space continues to be held properly.
