Obstructive sleep apnea happens when the airway repeatedly narrows or closes during sleep, interrupting breathing. Loud snoring, gasping, morning headaches and daytime fatigue are common signs. Because it can affect overall health, diagnosis is made through a physician and a sleep study. For some people with a confirmed diagnosis, a custom oral appliance that repositions the jaw may be recommended.
Silver Hill patients who have been diagnosed with sleep apnea, or whose physician has suggested an oral appliance, can discuss this option at our Port Dover office. The dentist will assess your teeth, gums and jaw to see whether an appliance is suitable and coordinate with your physician on follow-up.

Diagnosis first
Loud, frequent snoring, pauses in breathing noticed by a partner, waking up gasping or choking, morning headaches, dry mouth on waking and excessive daytime sleepiness can all suggest sleep apnea. High blood pressure and difficulty concentrating may also be linked. The dentist may notice signs during a checkup, such as tooth wear or a narrow airway.
If sleep apnea is suspected, your physician can arrange a sleep study and confirm the diagnosis and severity. Treatment options may include positive airway pressure therapy, lifestyle changes, oral appliances or other approaches. Oral appliances are generally considered for people with certain types and levels of sleep apnea, as determined by medical assessment.
Is an appliance suitable
An oral appliance relies on your teeth for support, so you need enough healthy teeth in both arches. The dentist examines your gums, existing dental work and jaw joints, and checks how far your lower jaw can comfortably move forward. X-rays and impressions or scans may be taken to plan the appliance and record your starting bite.
People with very few teeth, active gum disease, significant jaw joint problems or certain bite issues may not be good candidates. Dental problems may need to be treated first. The dentist will explain whether an appliance is likely to be suitable and work with your physician if another treatment would be better.
Fitting the appliance
The appliance is custom-made from impressions or scans of your teeth. It typically fits over both upper and lower teeth and holds the lower jaw slightly forward during sleep, which can help keep the airway open. At the fitting appointment, the dentist checks comfort and fit and shows you how to insert, remove and clean it.
The jaw position is usually adjusted gradually over several weeks to find a balance between effectiveness and comfort. You may notice jaw soreness, extra saliva or a changed bite in the morning at first. These often settle. Keep follow-up appointments so the dentist can fine-tune the appliance and check for side effects.
Ongoing care
Your physician may recommend a follow-up sleep study to check how well the appliance is controlling your sleep apnea. Report any return of snoring, fatigue or other symptoms to both your physician and dentist. Treatment needs can change with weight, age or health, so ongoing monitoring is important.
Clean the appliance daily as instructed and store it in its case. Bring it to dental checkups so its fit and condition can be checked. Long-term use can sometimes shift the bite, so the dentist will watch for changes. Report jaw pain, loose teeth or a cracked appliance promptly.
Planning your appointment
If your physician has recommended an oral appliance, contact our Port Dover office and mention your diagnosis. Ask your physician or sleep clinic to send the sleep study results before your visit.
Bring a list of medications and any previous appliances. If you use a CPAP machine, mention how it has been working for you.

A dentist may notice signs that suggest sleep apnea, but diagnosis is made by a physician, usually through a sleep study. Once diagnosed, a dentist can help with oral appliance therapy if it is recommended and suitable for you.
Effectiveness varies by person and severity. Oral appliances can help some people with certain types of sleep apnea, while CPAP is often recommended for more severe cases. Your physician will advise which treatment is most appropriate for your diagnosis.
Some jaw soreness, extra saliva or a changed bite in the morning are common at first and often settle with time. The dentist adjusts the appliance gradually to improve comfort. Report persistent pain or bite changes so they can be addressed.
With proper care, many appliances last several years. Wear, changes in your teeth or dental work and adjustments over time may eventually require a replacement. The dentist will check its condition at regular visits.
Over-the-counter devices are not custom-fitted or adjustable and are not a substitute for medically supervised treatment of sleep apnea. If you snore loudly or have symptoms of sleep apnea, see your physician for assessment first.
A custom oral appliance may quiet simple snoring, but sleep apnea must be ruled out first. Silver Hill patients can learn how fitting and follow-up work.
A night guard made from your bite can protect teeth from grinding. See how custom guards are made in Port Dover and how Silver Hill patients care for them.
