Obstructive sleep apnea occurs when the airway repeatedly narrows or closes during sleep, interrupting breathing and disturbing rest. It is associated with loud snoring, daytime sleepiness and other health risks. Diagnosis requires a sleep study arranged through a physician. For some people, especially with mild to moderate apnea or who cannot tolerate CPAP, a custom oral appliance may be one treatment option.
Hillcrest patients who have been diagnosed with sleep apnea, or whose physician has recommended an oral appliance, can discuss this at our Port Dover office. The dentist will assess your teeth and jaw and explain how an appliance works alongside your medical care, including how progress is checked with your physician.

Diagnosis first
Snoring alone does not confirm sleep apnea, and sleep apnea can occur without obvious snoring. A sleep study, arranged by your physician, measures breathing, oxygen levels and sleep patterns to determine whether apnea is present and how severe it is. This information guides treatment decisions. The dentist will usually need a diagnosis and a recommendation from your physician before making an appliance.
Loud snoring, pauses in breathing noticed by a partner, waking gasping or choking, morning headaches, daytime sleepiness, difficulty concentrating and high blood pressure can all be associated with sleep apnea. If you notice these, speak to your family doctor. The dentist may also raise concerns if they see signs such as worn teeth or a narrow airway during an exam.
Is it suitable?
Oral appliances attach to the upper and lower teeth, so enough healthy teeth and stable gums are needed to hold them. The dentist checks for decay, gum disease, loose teeth and jaw joint problems. Existing TMJ pain may affect suitability. X-rays and impressions or scans may be taken. Any dental issues that could interfere are discussed and may need treatment first.
Appliances are often considered for mild to moderate obstructive sleep apnea, or for people with more severe apnea who cannot use CPAP, in consultation with their physician. Your preferences, lifestyle and ability to wear the device nightly also matter. The dentist will explain the realistic expectations and limitations so that you and your physician can decide together.
The appliance
Most oral appliances for sleep apnea, called mandibular advancement devices, hold the lower jaw slightly forward during sleep. This can help prevent the tongue and soft tissues from collapsing into the airway. The appliance is custom-made from impressions or scans of your teeth and is usually adjustable so the position can be fine-tuned over time.
At the fitting appointment, the dentist checks comfort and fit and shows you how to insert, remove and clean the appliance. The jaw position may be adjusted gradually over several visits. A follow-up sleep study or assessment by your physician is often recommended to confirm that the appliance is effective for your condition and that no further changes are needed.
Ongoing care
Some people notice jaw stiffness, tooth soreness, excess saliva or dry mouth when starting to use an appliance. These often improve with time. Over the long term, appliances can cause changes in the bite, which the dentist will monitor. Report persistent jaw pain, changes in how your teeth fit together or damage to the appliance promptly.
Regular dental visits allow the dentist to check the appliance, your teeth and your bite. Your physician may also want periodic reviews of your sleep apnea. If symptoms such as snoring or daytime sleepiness return, contact both offices. Weight changes, new medications and ageing can affect sleep apnea, so treatment may need reassessment over time.
Planning your appointment
Call our Port Dover office if your physician has recommended an oral appliance or you have a diagnosis of sleep apnea. Ask your physician’s office to send the sleep study results so the dentist can plan an assessment.
If you have not been diagnosed but suspect sleep apnea, speak to your family doctor first about arranging a sleep study.
Before you book
Have a question about your situation? Our team can help you plan a visit.
No. Sleep apnea is diagnosed through a sleep study arranged by a physician. The dentist may notice signs and suggest you speak with your doctor, and can provide an oral appliance once a diagnosis and recommendation are in place.
CPAP is often more effective for severe sleep apnea, but some people find it difficult to use. Oral appliances can be effective for many people with mild to moderate apnea. Your physician and dentist can help you weigh the options.
Long-term use can cause gradual changes in how the teeth fit together. The dentist monitors for these changes at follow-up visits and may suggest exercises or adjustments. Report any persistent bite changes or jaw discomfort promptly.
Lifespan varies with material and wear, but many last several years with proper care. The dentist checks the appliance regularly and will recommend replacement if it becomes damaged, worn or no longer fits your teeth properly.
Some devices look similar, but sleep apnea appliances are prescribed and adjusted as part of medical treatment. Using a snoring device without a diagnosis may mask apnea symptoms. Speak with your physician if you suspect sleep apnea.
A custom oral appliance may reduce snoring for some people once sleep apnea is ruled out. Hillcrest patients can learn about fitting and follow-up visits.
A custom night guard can cushion teeth from clenching during sleep. Hillcrest patients can learn about fitting, comfort and care at our Port Dover office.
