Pain that will not settle

A severe toothache is a signal to find and treat the cause.

Severe tooth pain can make it hard to eat, sleep or concentrate. It may be sharp, throbbing or constant, and it may spread to the jaw, ear or head. Common causes include deep decay reaching the nerve, a cracked tooth, an abscess, a damaged filling or gum infection. Pain relievers may help for a while, but they do not fix the problem underneath.

If you are in Renton and dealing with a bad toothache, call our Port Dover office to describe the pain and any swelling or fever. The team will advise on interim relief and discuss how soon you should be seen, based on your symptoms and the day’s availability.

Understanding the pain

The pattern of pain offers useful clues.

Common causes

Deep cavities, cracked teeth, failing fillings or crowns, infected nerves, abscesses, gum infections and impacted wisdom teeth are frequent sources of severe toothache. Sometimes sinus problems or jaw muscle tension can mimic tooth pain. The dentist’s job is to work out which of these is responsible so the treatment can address the real source of pain.

Clues from the type of pain

Lingering pain after hot or cold may suggest an inflamed nerve. Pain when biting can point to a crack or infection around the root. Throbbing that wakes you at night often indicates more advanced inflammation. Swelling, a bad taste or fever suggests infection. Note these details before calling so you can describe them clearly to the team.

Interim relief

Comfort measures can ease pain while you wait.

  • What may help

Take an over-the-counter pain reliever that is safe for you, following package directions or a pharmacist’s advice. Rinse gently with warm salt water. Floss carefully around the tooth in case food is trapped. A cold compress on the cheek may help with swelling. Sleep with your head slightly raised if lying flat makes throbbing worse.

  • What to avoid

Do not place aspirin or other tablets directly on the gum, as this can burn the tissue. Avoid applying heat to the face. Steer clear of very hot, cold or sugary foods and chewing on the painful side. Do not take leftover antibiotics. If pain becomes unbearable or swelling spreads, seek urgent care right away.

Diagnosis and treatment

Treatment depends on what the dentist finds.

Finding the source

The dentist asks about the pain, examines the area and usually takes X-rays. Tests such as cold application, gentle tapping or biting on an instrument help identify the affected tooth and its condition. Sometimes pain is referred from another tooth, so a careful examination is important before deciding on treatment. Point to where it hurts most.

Treatment options

Depending on the cause, treatment may include a filling, removing an inflamed nerve and starting a root canal, draining an abscess, treating a gum infection or extracting a tooth that cannot be saved. Temporary measures may be used for immediate relief, with definitive care scheduled afterward. The dentist will explain the options and what each involves.

After treatment

Relief is often quick once the cause is treated.

  1. Following instructions

Some tenderness after treatment is normal and usually eases within days. Take medications as directed, including any antibiotics. Avoid chewing hard foods on the treated side until it has been fully restored. Contact the office if pain worsens, swelling develops or you have concerns about your recovery. Seek emergency care if breathing or swallowing is affected.

  2. Completing the plan

Many toothaches require follow-up treatment, such as completing a root canal or placing a crown. Keep these appointments to prevent the problem from returning. Regular checkups can catch decay or cracks before they cause severe pain, which may spare Renton patients another urgent trip to the office. Mention small twinges early rather than waiting for them to worsen.

Toothache questions

Have a question about your situation? Our team can help you plan a visit.

Can a severe toothache go away on its own?

Sometimes pain fades if the nerve dies, but the underlying problem, such as infection, usually remains and can worsen. Have the tooth assessed even if the pain eases, so the cause can be identified and treated properly.

Should I go to the hospital for a toothache?

Go to the hospital or call 911 if you have difficulty breathing or swallowing, rapidly spreading swelling or a high fever with feeling very unwell. For tooth pain without these signs, call the dental office for advice and an appointment.

Why does my toothache get worse at night?

Lying down can increase blood pressure in the head, which may intensify throbbing from an inflamed tooth. There are also fewer distractions. Keeping your head slightly elevated may help until you can be seen by the dentist.

Will I need a root canal?

Not always. The treatment depends on the cause. A cavity may need a filling, while an inflamed or infected nerve may require a root canal or extraction. The dentist will explain after examining the tooth and reviewing X-rays.

Can I take antibiotics for a toothache?

Antibiotics are not effective for all toothaches and are prescribed only when appropriate, such as for a spreading infection. They usually work alongside dental treatment rather than replacing it. Do not take leftover antibiotics without professional advice.
