Between a filling and a crown

Sometimes the best repair covers only what is damaged.

Inlays and onlays are custom-made restorations, often of ceramic or composite, that are bonded into or onto a back tooth. An inlay fits within the grooves of the biting surface, while an onlay extends over one or more cusps. They can be a middle ground for teeth with damage too large for a regular filling but not extensive enough to require a full crown.

If you are in Hillcrest and have been told a tooth needs more than a filling, our Port Dover office can explain whether an inlay or onlay is suitable. The dentist will compare it with a filling and a crown so you understand how much tooth each option preserves.

When they fit

These restorations suit a particular range of damage.

Teeth that may benefit

Inlays and onlays are often considered when a large old filling needs replacing, a cusp has cracked or broken, or decay has removed a significant part of the biting surface while leaving the outer walls reasonably strong. Because they are made outside the mouth and bonded in place, they can restore shape precisely and reinforce what remains of the tooth.

When a crown is still better

If most of the tooth is missing, the walls are very thin or the tooth has had root canal treatment and needs full protection, a crown may be the safer choice. Teeth under heavy grinding forces may also need more coverage. The dentist will explain why one option is preferred, so you understand the reasoning rather than just the recommendation.

Comparing options

Understand what each choice means for the tooth.

Preserving natural tooth

Compared with a crown, an onlay usually requires removing less healthy tooth structure, because only the damaged areas are covered. That can be an advantage over the long term, since every restoration may eventually need replacing. Ask the dentist how much tooth would be removed for each option and how that affects future treatment possibilities.

Materials, time and cost

Ceramic inlays and onlays are tooth-coloured and resist staining, while gold remains an option for some patients who prefer its durability. Laboratory-made restorations usually take two visits and may cost more than a direct filling. Ask for a written estimate and check your insurance coverage directly so the financial side is clear before you commit.

The appointments

Treatment is usually completed in two stages.

  1. Preparing the tooth

At the first visit, the tooth is numbed, old filling material and decay are removed and the tooth is shaped. Impressions or a digital scan are taken so a laboratory can make the restoration. A temporary filling protects the tooth in the meantime. Avoid sticky or very hard foods on that side until the permanent restoration is placed.

  2. Bonding the restoration

At the second visit, the temporary is removed and the inlay or onlay is tried in to check fit, contacts and colour. Once everything looks right, it is bonded to the tooth and the bite is adjusted. The surface is polished smooth. Some sensitivity can occur for a short while as the tooth settles after treatment.

Looking after it

Good care helps the restoration last for years.

  • Everyday habits

Brush twice daily with fluoride toothpaste and clean between teeth carefully, especially along the edges where the restoration meets the tooth. Limit frequent sugary snacks, which increase the risk of decay around margins. Avoid chewing ice or hard objects. If you clench or grind, a night guard may protect both the restoration and the remaining tooth structure.

  • Monitoring at checkups

At routine visits, the dentist checks the margins, the bond and the surrounding tooth for any signs of chipping, wear or decay. Contact the office if the restoration feels rough, your bite changes or the tooth becomes sensitive to biting. Small issues found early can often be addressed before the tooth needs more extensive work.

Planning your appointment

Getting an inlay or onlay planned in Port Dover.

Hillcrest patients can call our Port Dover office to book an assessment of a damaged or heavily filled tooth. Mention any recommendation you have already received so the dentist can review the options with you in detail.

Ask how far apart the two visits are likely to be, so you can plan travel and avoid hard foods while the temporary is in place.

Inlay and onlay questions

Have a question about your situation? Our team can help you plan a visit.

What is the difference between an inlay and an onlay?

An inlay fits inside the grooves of a back tooth, similar to a filling. An onlay extends over one or more cusps to protect weakened areas. The choice depends on how much of the tooth is damaged and which parts need support.

Are inlays and onlays stronger than fillings?

Laboratory-made restorations are often more durable and wear-resistant than large direct fillings, and they can reinforce the tooth. However, they are not always necessary. The dentist will explain whether the added strength justifies the extra visit for your tooth.

How long do inlays and onlays last?

Many last for years with good care, although lifespan depends on material, bite forces and oral hygiene. Regular checkups help identify wear or decay around the edges early, allowing repair before larger problems develop in the tooth underneath.

Will the restoration match my tooth colour?

Ceramic and composite inlays and onlays can be shaded to blend with your natural tooth. Gold restorations are visible but very durable. The dentist will discuss which material suits the tooth location and your preferences.

Is an onlay better than a crown?

An onlay preserves more natural tooth, which can be an advantage, but a crown offers fuller protection when much of the tooth is weakened. Neither is universally better. The right choice depends on the condition of your tooth.
