When a tooth must come out

Removing a tooth is a decision worth understanding fully.

Teeth may need to be removed because of severe decay, advanced gum disease, a fracture below the gum, infection that cannot be treated another way, crowding before orthodontics or problems with wisdom teeth. Saving a natural tooth is often preferred when it is possible, so the dentist will explain why an extraction is being recommended and what alternatives, if any, exist.

For patients from Fishers Glen, an extraction at our Port Dover office begins with an exam and X-rays. The dentist will review your health history, discuss what removal involves and talk through ways to replace the tooth afterward if needed. You will have the chance to ask questions before agreeing to anything.

Making the decision

Understand the reasons before agreeing to removal.

When saving the tooth is not realistic

If a tooth is broken beyond repair, too loose from gum disease or has an infection that root canal treatment cannot reliably resolve, extraction may be the most predictable option. In other cases, it may be one choice among several. The dentist should explain the outlook for each path so you can weigh them against your health, priorities and budget.

Questions to ask

Ask whether the tooth could be saved with a root canal and crown, what happens if you wait and how removal will affect your bite. If the tooth is visible, ask how the gap will be managed in the short term. Taking time to understand helps you feel confident in the decision, and a second opinion is always reasonable for a non-urgent extraction.

Preparing

Share full health details ahead of time.

  • Health and medications

Tell the dentist about all medications, especially blood thinners, bone medications and supplements. Mention heart conditions, joint replacements, diabetes or immune concerns. Some situations require coordination with your physician before the extraction. Never stop prescribed medication on your own, even if you think it may affect bleeding, as that can create other risks. The team will advise you.

  • Planning the day

Eat a light meal beforehand unless instructed otherwise, and arrange to rest afterward. If sedation is planned, you will need someone to drive you home. Have soft foods and any recommended pain relievers on hand before you leave Fishers Glen for the appointment, so you can go straight home and rest without needing to stop on the way.

The procedure

Most extractions are done with local anaesthetic.

  1. Numbing and removal

The area is numbed thoroughly. You may feel pressure as the tooth is loosened, but you should not feel sharp pain. Tell the dentist if you do, and more anaesthetic can be given. Some teeth, particularly those that are broken or have curved roots, may need to be sectioned or require a small incision for removal. The dentist will explain each step.

  2. Closing the site

Once the tooth is out, the site is cleaned and gauze is placed to help a blood clot form. Stitches may be used in some cases, and the dentist will tell you whether they dissolve or need removing. The dentist will go over aftercare instructions before you leave, and you can ask for them in writing to refer to at home.

Healing

Protect the clot and follow aftercare closely.

The first few days

Bite on gauze as directed and avoid spitting, using straws or smoking, as these can dislodge the clot and lead to a painful dry socket. Stick to soft foods and chew on the other side. Mild swelling and discomfort are normal. Call the office if bleeding does not slow, pain worsens after a few days or you develop a fever.

Replacing the tooth

Once healing is underway, discuss whether and how to replace the tooth. Options may include an implant, bridge or partial denture. For back teeth, a gap can sometimes allow neighbouring teeth to shift. The dentist can explain the timing and choices that suit you, including which options need the site to heal fully first. There is no rush to decide.

Extraction questions

Have a question about your situation? Our team can help you plan a visit.

Does having a tooth pulled hurt?

The area is numbed so you should feel pressure rather than pain during removal. Some soreness afterward is normal and can usually be managed as the dentist advises. Report severe or worsening pain, particularly if it begins a few days after the extraction.

What is a dry socket?

A dry socket happens when the blood clot is lost from the extraction site, exposing bone. It can cause significant pain a few days after removal. Avoiding straws, spitting and smoking helps prevent it. Call the office if you suspect one, as it can be treated.

How long does healing take?

Initial healing of the gum often takes a week or two, while the bone continues to remodel for longer. Recovery varies by the tooth and your health. Follow the dentist’s instructions and attend any follow-up appointment.

Can I go back to work the same day?

Many people can return to light activity soon after a simple extraction, but rest is recommended for the remainder of the day. If sedation was used, you should not drive or work until it has fully worn off, and someone should stay with you.

Do I need to replace the extracted tooth?

Not always, but replacing it may help with chewing, appearance and preventing teeth from shifting. The dentist can explain the pros and cons of each option for your situation, and whether waiting would affect the choices available later.
