An infection to take seriously

An abscess will not clear up on its own and should be seen soon.

A dental abscess is a pocket of infection, usually at the tip of a tooth root or in the gum beside a tooth. It can cause throbbing pain, swelling, a bad taste or a pimple-like bump on the gum. Sometimes pain eases if the abscess drains, but the infection is still present and needs treatment.

If you are in Boston and think you may have an abscess, call our Port Dover office promptly. Describe the swelling, pain and any fever, and the team will advise how soon you should be seen and whether you need to seek medical care at a hospital first.

Recognising it

Pain and swelling are the most familiar warning signs.

  • Common symptoms

Persistent throbbing toothache, tenderness when biting, swelling of the gum or face, a bump on the gum that may leak fluid, a foul taste and sensitivity to hot drinks are typical signs. Some people also feel generally tired or unwell. Any of these justify calling the office rather than waiting for your next routine visit, because infections tend to worsen without treatment.

  • Signs of a spreading infection

Swelling that is growing rapidly, spreading under the jaw or toward the eye, fever, difficulty opening the mouth, or trouble swallowing or breathing can signal a more serious infection. These require urgent medical attention at an emergency department, not a wait for a dental appointment. Do not drive yourself if you feel very unwell; call for help instead.

Before your visit

Comfort measures help, but they do not treat the infection.

  1. Managing discomfort safely

Rinsing gently with warm salt water may soothe the area. Over-the-counter pain relief may help if it is safe for you; follow the label directions and ask a pharmacist if you have health conditions or take other medicines. A cold compress on the outside of the cheek can ease swelling when applied for short periods at a time.

  2. What to avoid

Do not try to pop or drain a swelling yourself, and avoid placing tablets or heat directly on the gum or face. Do not cancel the appointment because pain has improved, since that can happen when an abscess drains without the infection actually clearing. Keep the scheduled visit so the source can be found and treated properly.

Treatment

Treatment focuses on removing the cause of the infection.

Draining and removing the source

Depending on the findings, the dentist may drain the abscess, perform root canal treatment to clean the inside of the tooth, or remove the tooth if it cannot reasonably be saved. Gum abscesses may need thorough cleaning of the affected pocket. X-rays help show the extent of infection and guide which of these approaches is most appropriate for you.

The role of antibiotics

Antibiotics may be prescribed in some situations, particularly when infection is spreading or you have certain health conditions. They are usually used alongside treatment of the tooth rather than instead of it, since they cannot remove the source. Take any prescription exactly as directed, finish the course and tell the dentist about any allergies beforehand.

Recovery

Watch the healing and finish the treatment plan.

The days after treatment

Some tenderness after treatment is common and usually eases over a few days. Swelling should gradually decrease rather than grow. If pain worsens, swelling returns or you develop a fever, contact the office promptly. Keep the area clean with gentle brushing, and follow any instructions you were given about rinsing, eating and medication in the days after the visit.

Follow-up care

If root canal treatment was started, further visits and a crown may be needed to protect the tooth from breaking. If a tooth was removed, discuss replacement options once healing is complete. Regular checkups help catch decay and gum problems early, before they have a chance to progress to another painful infection down the road.

Planning your appointment

Abscess questions

Have a question about your situation? Our team can help you plan a visit.

Can a dental abscess go away without treatment?

No. Even if the pain eases, the infection usually remains and can spread to surrounding tissues. An abscess needs dental treatment to remove the source of infection, whether that means drainage, root canal therapy or extraction.

Will antibiotics alone fix an abscess?

Antibiotics may help control the spread of infection but generally do not resolve the cause. Treatment such as drainage, root canal therapy or extraction is usually needed as well, otherwise the infection is likely to return.

When should I go to the emergency room?

Go to an emergency department if swelling is spreading quickly, you have a high fever, or you have difficulty breathing, swallowing or opening your mouth. These signs suggest a serious infection that needs medical care right away.

Is it safe to put heat on the swelling?

Heat is generally not recommended, as it may encourage swelling to spread. A cold compress on the outside of the cheek for short periods is a safer comfort measure while you arrange to be seen by the dentist.

Can the tooth be saved?

Often, with root canal treatment followed by a crown. Sometimes the tooth is too damaged and extraction is advised instead. The dentist will explain the options after examining the tooth and reviewing the x-rays with you.
